  1. This study aimed to develop a comprehensive strategy package to enhance implementation of post-discharge information summary (PDIS) for improving patient experience and health-related quality of life.
  2. Factors associated with healthcare professionals such as knowledge of PDIS, agreement on roles and responsibilities, attitudes towards the intervention, implementation intentions, and goal setting were emphasised for post-discharge self-management among hospitalised older patients.
