- This study aimed to develop a comprehensive strategy package to enhance implementation of post-discharge information summary (PDIS) for improving patient experience and health-related quality of life.
- Factors associated with healthcare professionals such as knowledge of PDIS, agreement on roles and responsibilities, attitudes towards the intervention, implementation intentions, and goal setting were emphasised for post-discharge self-management among hospitalised older patients.
